Paperwork & Physical Information
As of September 1st, 2021, we will no longer be accepting paper physicals. Please keep the hard copy for your own files and upload the front and back of the physical to your Aktivate Account.
To be cleared for participation in sports, EVERY YEAR you must 1. Update your Aktivate Account and fill out the appropriate forms AND 2. Upload the Front (medical history and parent/guardian consent signature) and Back (physician evaluation and signature) of the IHSAA Physical Exam and Consent Form your Freshmen and Junior years. If you are new or a transfer student, you can upload your current physical if it was completed after May 1st going into your Freshmen and Junior year. Without this clearance, you will not be able to try out or participate in practices until these are all completed.
