Online Meridian Schools Clinic Registration:
The Meridian Schools Clinic is designed to keep kids healthy and learning in the West Ada School district. Services can be accessed by children when parents cannot be physically present if the parent fills out the Consent to Treat and Privacy documents. They can be filled out online and signed by parents using this link so they do not have to leave work or other child commitments:

It’s almost impossible to predict illnesses or minor injuries during the school day, so West Ada School District and Family Medicine Health Center have teamed together to provide your child the healthcare they may need during the school day when you can’t be there.
How It Works:
If your child is not feeling well and they see the school nurse, the nurse may refer your child to Meridian Schools Clinic and will coordinate with you as the parent/guardian in doing so. During this process, Parent Consent Forms must be on file in order to send your child out to the clinic; this includes consent to treat your child at MSC, consent to share your child’s health information between the school and MSC, and consent to transport your child to-and-from school (if it’s a participating school).
Once a parent consent form is on-file with the school nurse, your child will have access to each of the following clinic services:
- Treatment of Minor Injuries and Illnesses
- Well-Child Exams
- Sports Physicals
- Behavioral Health
- Medications and/or Prescriptions
- Dental Hygiene Services: Fluoride Varnish, Sealants, and Cleanings
We accept most insurances. If you are without health insurance, we offer discounted rates for families based on their income level and family size – No one will be refused care for the inability to pay.

For your child(ren)’s first visit, you will be asked to provide:
- Health history
- Immunization Records
- MSC patient registration forms, if not already on file
- Insurance / Medicaid card if eligible
Should your child get ill during the school day, your school nurse or one of the MSC staff will attempt to contact you to coordinate care.
