You will receive a follow-up email (in a few days) with additional information on how to complete the online medical and demographic forms. These forms MUST be completed before you can make a registration appointment.
Bring the following documents to Andrus Elementary BEFORE the appointment.
Birth Certificate (original/official ~ photocopies will not be accepted)
Immunization Record (hard copy)
Proof of Address (utility bill or purchase/rental agreement only)
Kindergarten Registration Date (by appointment only)
