Have you completed the new ‘Consent to Treat’ form in Powerschool to allow our Health Office to treat your child in the event of a minor injury here at school?
Without that PowerSchool consent, Health Tech Sabina or Nurse Jeneille are NOT allowed to offer treatment. Please email Health Tech Sabina (selimovic.sabina@westada.org) or Head Secretary Jamie Gates (gates.jamie@westada.org) OR call PS Helpline (#208-350-5300) if assistance is needed to find and complete that form in PowerSchool this week.
