All information submitted on this form is transmitted securely and kept private by the Education Foundation of the West Ada School District, a private, nonprofit 501(c)3 organization (EIN # 82-0421800) established to provide supplemental funding for projects and programs in the West Ada School District.

*Please put the first and last name of the teacher, as donations go to a specific teacher's classroom fund. Please designate to a teacher, not a specific project. Designate to Mrs. Smith's classroom not Mrs. Smith's art project.
