Before your appointment is scheduled all registration information will need to be completed.
· On-line Registration
· Complete PowerSchool (instructions will be emailed once you complete on-line registration)
· Birth Certificate
· Immunization Records

· Proof of Residency
A short screening will be completed during the appointment.
