Note: All enrollment information needs to be completed prior to scheduling your appointment.
Here are the following items that need to be completed:
+On-line Enrollment
+Complete PowerSchool (instructions will be emailed once you complete on-line enrollment)
+Student Enrollment Document Upload
Birth Certificate
Immunization Records
Proof of Residency
Proof of Immunization Status – For additional information regarding immunization status please check the Health Services website: WASD Health Services
A short screening will be completed during the appointment.
